Diagnosis and management of postpneumonectomy empyema with an Eloesser flap.
Postpneumonectomy empyema (PPE) is an uncommon and devastating complication of pneumonectomy, with substantial morbidity and mortality. The condition is characterized by pleural cavity infection and inflammation. This article focuses on the management of complicated and persistent PPE with a procedure called Eloesser flap placement, a type of open-window thoracostomy.

In 13 patients an open window thoracostomy (OWT) was performed for post pneumonectomy pleural empyema. The operation, and life with an OWT cavity, were tolerated well. Early closure of an OWT is not advisable because of a high chance of recurrence of the infection and, in lung cancer patients also the risk of tumour relapse within two years after tumour surgery.

Pilomatrix carcinoma originates in the matrix cell and is marked by exophytic growing and common local recurrence. There is no established treatment for wide local invasion and metastasis of the pilomatrix besides wide surgical resection. We report a case of rapidly progressive pilomatrix carcinoma, which arose around an Eloesser open window with direct invasion to the adjacent tissue.
